Well Child ROS Section
This section allows the provider to record information during the "review of systems" portion of the encounter. This section appears in the encounter only for patients under 22 years old.

During the Exam stage of the encounter, providers access this section in the ROS step.


- From the Exam stage of the patient encounter, locate the Well Child ROS section.
- Check the checkbox for the Well Child ROS template you want to appear for this patient encounter, or click the set to last encounter link to select the same template as the last encounter for this patient. The elements of the selected template appear.
- Click the appropriate elements and items, and enter any free-text notes in the text-entry box at the far right of the element.
- You can click
the downward facing triangle
to the left of the relevant paragraph name to mark all findings in that paragraph as normal. Within that paragraph you can continue to mark abnormal findings.
- You can click
the plus icon
to expand paragraphs; click the minus
icon to contract or roll up paragraphs.
Note: You cannot contract paragraphs after you make documentation notes within them. - Click Save.

- Display the HPI, ROS, or PE section of the encounter.
- Document the section as usual.
athenaOne saves any notes or free text that you enter in the template in the saved findings.
If you select multiple templates, athenaOne saves all selected templates under one saved finding set.
Note: Jotter images are not preserved in the saved findings when you save findings in the encounter. - Click the star
icon to the right of the section name (for example, Review of Systems) to save the findings for later use.
- Name — Enter a name for the saved findings (up to 50 characters).
Note: Names can be used only once per user. - Click Save.
A confirmation message appears.

- Display the HPI, ROS, or PE section of the encounter.
- Use saved findings — Select the saved findings that you wish to apply.
Findings saved by the current user and the rendering provider for the appointment are available for selection. The username of the user who created the saved findings appears in parentheses after the name of the saved findings.
Note: Click the plusicon to search for saved findings or previous encounter templates. A list of your commonly used templates and saved findings for this type of encounter and patient history appears.

Make changes and notes to reflect the current patient's visit. (These changes will not affect the saved findings you just applied.)
Note: Selecting a saved finding replaces any previously entered documentation in that section. - Click Save.

The contents of this section is fully customized for each practice via the Well Child Review of Systems Templates page.
When the provider signs off and saves the encounter, the patient's chart is automatically updated with the information recorded in this section.
If the summary counterpart to this section (Review of Systems Summary) is included in the encounter form, the information recorded in this section is automatically added to the encounter summary when the provider signs off and saves the encounter.

Defaults to "Parent" for patients under age 10. For patients age 10 and over, this option defaults to "Patient."
You can also enter free text (for example, "Aunt Betty reports") for any option selected from the reported by menu to change the selection. Move the cursor into the text box to enter text. The text you enter will appear in the encounter summary as "TEXTENTEREDBYUSER reports…"
Please do not use this field for notes because the narrative for the ROS is designed to accommodate a noun only. Using this field to document general notes will cause the encounter summary to narrate incorrectly. |
Set to previous encounter |
This auto-complete field appears only if there are any previous encounter dates where information was recorded in this encounter section.
The list includes previous encounter dates (in the past 18 months) where information was recorded in this encounter section. If you select an encounter from this list, the jotter images, values, and findings recorded during the selected encounter date are used to populate the template for the current encounter.
If you change the selection, the template is cleared and reset to the new choice. |
